California's Affordable Insulin Initiative Faces Slow Adoption Despite Early Progress

In San Francisco's Japantown, pharmacist Margaret On is prepared to support patients in need with California's new affordable insulin brand, CalRx. Although she hasn't yet dispensed any, On sees the potential benefit for uninsured individuals.

Since its launch seven months ago, CalRx has distributed over 120,000 five-pen packs of insulin glargine, priced at $55 each—substantially lower than the up to $411 charged by leading brand names. This initiative marks a significant move by California, as it competes with industry giants Eli Lilly, Sanofi, and Novo Nordisk for the first time.

Governor Gavin Newsom's CalRx program aims to provide essential medications to those who can't afford them and disrupt the pharmaceutical market. As he approaches the end of his second term, Newsom plans to focus on healthcare in his potential 2028 presidential campaign. The state has partnered with Civica, a nonprofit, for the development of the insulin and aims to expand its generic drug offerings, including medications for asthma and severe allergic reactions, before he leaves office.
