Medicare Expands Pilot Program for Chronic Disease Management Technology

Medicare is set to broaden its innovative program aimed at leveraging technology for chronic disease management, just months after its initial launch.
The ACCESS model, introduced last year, incentivizes approved companies to assist patients in managing a variety of chronic health issues, including diabetes, hypertension, chronic pain, depression, and anxiety. A key feature of this model is its outcome-aligned payment structure, which rewards healthcare providers for improving patient health outcomes.
In a recent announcement, officials revealed plans to add new chronic conditions into the ACCESS program. Starting in spring 2027, the model will also cover substance use disorders, heart failure, chronic obstructive pulmonary disease (COPD), tobacco use, and extended support for musculoskeletal conditions.
